EEO & AD Issues Resolution Request Form

This form should be completed by employees and volunteers who seek resolution of an issue as defined in Orange Sky's Equal Employment Opportunity & Anti Discrimination (EEO & AD) Policy.

The issues resolution process applies to issues that arise in the course of work you do with Orange Sky, and other issues you wish to address. We encourage all employees and volunteers to try to resolve the issue directly, but will ensure that issues tabled through this process will be treated seriously and handled in a careful and confidential manner.
